Once Again, Our Annual Contest Was a Hit!

Here at Liberty Oxygen and Medical Equipment, we understand that medical supplies and oxygen equipment normally isn’t very exciting so we try to have a little fun with it and one way is hosting the annual Show Us Your Crown photo contest. Liberty Oxygen donates to the American Lung Association Minnesota based on the number of entries we receive. With each entrant who submits a photo (or multiple photos) to the contest while wearing one of our Lady Liberty crowns, Liberty Oxygen and Medical Equipment donates $10 to the Minnesota chapter of the American Lung Association.

This year marked our 11thyear hosting the annual Show Us Your Crown contest and once again we received many photo entries, and it looks like everyone is having fun with it! Each year, our friend Cheryl Sasse, RRT, Director, Lung Health, from the American Lung Association Minnesota is the official judge and chooses the winning photos from all the entries. Sleep Apnea

These articles will provide some background information for each and help you understand how our oxygen and medical equipment resources may be best suited for you or your senior loved one’s needs.

Sleep apnea is defined as a potentially serious sleep disorder in which an involuntary stoppage of breathing occurs while a person is sleeping. An individual may have sleep apnea if loud snoring and not feeling completely rested even after a full night’s sleep is occurring. There are three main types of sleep apnea:

  • Obstructive sleep apnea– This is the most common form and occurs when the throat muscles relax.
  • Central sleep apnea– This type of sleep apnea occurs when your brain is not sending the proper signals to the muscles which control breathing.
  • Complex sleep apnea– Also referred to as treatment-emergent central sleep apnea, this type occurs when a person has both central and obstructive sleep apnea.

Loud snoring is an indicator of a potentially serious problem, but not everyone with sleep apnea snores. Individuals with untreated sleep apnea will stop breathing several times a night, sometimes hundreds of times. This leads to the brain and body not getting enough oxygen. In addition to loud snoring, common symptoms include:

  • Morning headaches
  • Waking up with a dry mouth and throat
  • Occasionally waking gasping or choking
  • Lack of energy during the day
  • Difficulty staying asleep
  • Irritability, forgetfulness and mood changes

There are treatment options, such as losing weight, adjusting sleep positions, dental devices, surgery, and CPAP therapy, which can ease the symptoms and help prevent heart problems and other complications that can result from sleep apnea. The most common treatment for sleep apnea is CPAP therapy. Continuous Positive Airway Pressure (CPAP) is a treatment where the individual wears a mask over the nose and mouth while sleeping. The CPAP mask is connected to a small machine that is delivering continuous air flow to the nose, which in turn encourages regular breathing. There is also BiPAP, or bi-level positive airway pressure. This is similar to CPAP the air flow changes as the person breathes in and out.

Chronic Obstructive Pulmonary Disease (COPD)

The human body’s lung system is complex and hard at work, breathing in oxygen and exhaling carbon dioxide thousands of times every day. Throughout this intricate system, lung disease can occur when problems arise and tens of millions of Americans suffer from a form of lung disease today. Oftentimes, people associate chronic lung disease with lung cancer but there are many different types. If the airways, lung tissue, or circulation of blood in and around the lungs is compromised, some type of lung disease is involved. Here are some common types:

  • Asthma
  • Chronic obstructive pulmonary disease
  • Pulmonary hypertension
  • Cystic Fibrosis
  • Pneumonia
  • Lung cancer

Today we are going to take a closer look at chronic obstructive pulmonary disease, commonly referred to as COPD. COPD is a chronic lung disease that affects the ability to fully exhale, thus making it difficult to breathe, and includes bronchitis and emphysema.

Typical warning signs of chronic obstructive pulmonary disease may include, but are not limited to:

  • Chronic cough
  • Wheezing
  • Shortness of breath
  • Chronic mucus or phlegm
  • Coughing up blood
  • Chronic chest pain

Is All Durable Medical Equipment Covered by Insurance?

The simple answer is no. Medicare Part B (medical insurance) covers medically necessary durable medical equipment (DME) that your doctor prescribes for use in your home. Your physician, nurse practitioner (NP), physician’s assistant (PA) or certified nurse specialist (CNS) can prescribe medical equipment for a patient. Medicare is considered the driver when it comes to coverage criteria for durable medical equipment and all other insurance companies typically follow their guidelines. Acceptable durable medical equipment must meet these criteria:

  • Durable and can withstand repeated use
  • Used for a medical reason
  • Would not be useful to a person who is not sick or injured
  • Used in the home
  • Expected lifetime of at least 3 years

Durable medical equipment is defined as certain medical equipment, such as a walker, wheelchair, or hospital bed, that is ordered by a doctor for use in the home. Also referred to as home medical equipment (HME), oxygen equipment and CPAP supplies and accessories are included in the group of covered items. Durable medical equipment Medicare will cover includes, but is not limited to:

  • Blood sugar monitors & test strips
  • Canes (not including white canes for the blind)
  • Commode chairs
  • Crutches
  • Hospital beds
  • Manual wheelchairs and power mobility devices
  • Nebulizers and nebulizer medications
  • Oxygen equipment and accessories
  • Sleep apnea and Continuous Positive Airway Pressure (CPAP) devices and accessories
  • Walkers

Medicare has changed the way it reimburses for durable medical equipment. Prior to the Prescription Drug Act of 2003, paperwork could be completed after the patient received the equipment and any provider could be used, but now all paperwork must be completed and approved prior to receiving equipment and only providers with a Medicare Competitive Bid can be used. Additionally, the patient must meet certain coverage criteria for the item. If the specific criteria are not met, insurance will not cover the item. Insurance companies have tightened the reigns on requirements for durable medical equipment and simply requesting a physician to write a letter for coverage is no longer a viable option. For an insurance company to cover a specific piece of medical equipment, the patient must complete each of the following:

  1. Meet the coverage criteria for the particular piece of equipment.
  2. Provide a detailed, written order from a physician, nurse practitioner, physician’s assistant or certified nurse specialist to the DME provider.
  3. Provide a note from the above clinician to the DME provider confirming an in-person visit as well as a discussion about the need for specific medical equipment took place.
  4. Provide any necessary documentation supporting the need for the specific equipment.

Insurance dictates that no medical equipment can leave the showroom or warehouse until the DME provider has all of the above documentation first. If the DME provider does not obtain the documents, insurance will not cover the item and the patient will be responsible for payment.

Liberty Oxygen has a list of local providers for reference and the www.medicare.govwebsite provides up-to-date, important information. The ability to search the list of local and national competitive bid providers is available on the Medicare website by entering your zip code and the item needed. Be aware if you are searching for a provider using the Medicare website, some providers appear to be local to Minneapolis and St. Paul metro area even if they are not. This is because the provider will ship items and this can be costly.

Parkinson’s Disease

These articles will provide some background information for each and help you understand how our oxygen and medical equipment resources may be best suited for you or your senior loved one’s needs.

One of the most well-known early signs of Parkinson’s disease is a slight tremor or shaking in your hand. A slight tremor may be normal if you are feeling stressed or after exercise, but if you’re experiencing this while at rest, this could be an early sign of Parkinson’s. There are other common signs, such as loss of smell, change in handwriting, trouble moving and even issues with sleeping that all point to Parkinson’s. Demonstrating any one of these or other signs alone does not necessarily mean you have Parkinson’s disease, but if you experiencing more than one it may be a good idea to see a physician.

The Mayo Clinic’s website defines Parkinson’s disease as a progressive disorder of the nervous system that affects movement. Often beginning on one side of the body, the symptoms develop gradually and worsen over time as your condition progresses. Although the symptoms and signs will vary from person to person, some common signs may include:

  • Tremor –a shaking often begins in a hand or fingers.
  • Bradykinesia (slowed movement) –over time the disease may reduce the ability to move or slow movement making typically simple tasks more difficult.
  • Rigid muscles –muscle stiffness may occur in any part of the body which can cause pain and limit range of motion.
  • Speech changes –slurred, hesitated or monotoned inflection are some speech problems that may occur as a result of Parkinson’s.
  • Writing changes –writing may become small or become hard to write altogether.
  • Loss of automatic movement –decreased ability to perform unconscious movements such as swinging your arms when walking or even blinking.
  • Impaired posture and balance –balance issues or stooped posture can result from the disease.

Although there is no cure for Parkinson’s disease, there are options to help manage the disease and progression, such as:

  • Exercise
  • Medication
  • Surgery
  • Physical therapy
  • Speech therapy
  • Occupational therapy

Although the cause is unknown, research shows that certain neurons in the brain (nerve cells) gradually break down or die in those who have this disease. Many of the signs and symptoms discussed here are triggered due to a loss of neurons that produce the chemical messenger in your brain called dopamine, which causes abnormal brain activity and leads to the signs of Parkinson’s disease. Other factors which appear include genetics and environmental triggers. In addition, researchers feel the most important clue to the cause of Parkinson’s is the presence of Lewy bodies in the brain. Lewy bodies are clumps of specific substances within brain cells. More information can be found on the Mayo Clinic website.

Bathroom Safety

We reached out to our friends at Above & Beyond Senior Services for some tips on bathroom safety. At Liberty Oxygen and Medical Equipment we realize the increased level of safety concern when an older adult is in the bathroom and recently asked Above & Beyond Senior Services to share their expertise to help our customers maximize their safety in the bathroom.

Bathrooms have an especially high level of safety concern that other areas in a home do not. As a physical therapist, I am frequently asked to assess a person’s bathroom for accessibility, and to make recommendations for equipment and modifications that will maximize safety.

If a person uses an assistive device for walking, such as a walker, it can’t always fit into the bathroom. Consider these ideas in this situation: 

  • Avoid adding clutter that may make the walking spaces more restricted,
  • Add grab bars if you need to leave your walker outside the room.

The surfaces in a bathroom are unforgiving. They become slippery when wet and are hard if you land on them.

  • Keep the floor free of rugs, especially if they have fringe,
  • Use a rubber-backed bath mat when getting out of the shower so the floor stays dry and slip resistant.

In most instances, a standard height toilet does not meet the needs for a person with an arthritic knee, hip or spine. And in some cases, a toilet may be too high for a short person or someone transferring from a wheelchair. Also, if given an option of which chair to sit in, would you choose one with armrests or without? The toilet area is no different. If you have difficulty getting clean due to limited arm/hand function, there may be help! Here are some ideas:

  • Elevate your toilet with a seat riser, or have an ADA (Americans with Disabilities Act) toilet installed, which is 19” tall,
  • Add a safety frame, which is simply a set of armrests that attach to the bolts of the toilet seat.
  • A portable bidet can be added that washes and dries!

Stepping into a shower or tub also presents some difficulty. Do you find yourself unsteady if you look up and/or close your eyes? This is typical movement in a shower but is not comfortable for many people. Consider these modifications:

  • Grab bars as you step in as well as inside the shower
  • Shower chair with back
  • Transfer bench that allows you to sit on the outside of the tub and slide across (to avoid stepping over the edge)
  • Hand held shower with a shut off on the showerhead
  • Rubber mat on the shower floor or a textured surface (non-slip strips or sanded paint)

Finally, all grab bars should be installed professionally. They need to be firmly attached. A towel bar or a suctioned handle will not provide enough stability!

Overall, physical strength and balance are critical factors in order to have safety in the bathroom and throughout your home. An on-going, 2-3 times per week, exercise program that specifically targets your balance is important. Consider implementing a maintenance physical or occupational therapy plan, we are one of several providers in the metro area.
